Use the information below to request a General Fund Facility Work Order for your lab space or adjacent areas (such as restrooms, hallways, etc.). Please report issues for other spaces (such as classrooms, conference rooms, and the main admin suite) to our administrative team at dhgadstaff@umich.edu.
Facility work orders are not to be used to report an emergency, such as a fire. Emergencies would still be reported by calling the Division of Public Safety & Security 734-763-1131 or dialing 911 from your cell phone.
Please notify Ruth Halsey if there is an emergency in your space or nearby.
Examples include: an overflowing toilet
Please cite Ruth Halsey (halseyr@umich.edu) as the approver.
Navigate to the Facilities website (https://requests.fo.umich.edu/)
Select “Maintenance & Facilities”
Always list Ruth Halsey (halseyr@umich.edu) in the ‘confirmations’ email
Examples include: a flickering or burned-out light, water leak, and restroom issues, among other examples below
* Cleaning requests (unexpected messes made inside the building needing service outside of normal custodial service periods)
* Toilet paper or paper towels in the bathrooms are out
* Grounds issue (mess outside that needs to be cleaned-up)
* Electrical outlet not working
* Power is out or flickering
* Door issue (won’t open, won’t close, won’t lock, etc.)
* Drain is clogged
* Unidentifiable smells from the ventilation
* Unusual sounds coming from the building
* Elevator issue
* Water leaks
* Water issues (discolored water, no water, water pressure problem, etc.)
* Water faucet not working
* Replacing light bulbs
* Toilet/bathroom issues
* Plumbing problem
* Heating/cooling issues
* Stained ceiling tiles (indicates a water leak)
You are authorized to submit ONLY “General Fund Work Orders”. Orders placed through “General Fund Work Orders” are not charged. Our unit pays for our space and included with our space costs are repairs for normal wear and tear that would require maintenance. Any items that are not the result of ‘normal wear and tear’ would qualify as “Non-General Fund Work Orders” and should be submitted or authorized in advance by Ruth Halsey. This proposed work would require Department leadership to evaluate the cost of the activity vs. the value added to our unit.
